The investigative and auditing arm of the U.S. Congress, renamed the Government Accountability Office in 2004. Two of its 1992 reports on the Patriot missile system's Gulf War performance anchor the vault's Patriot cluster: IMTEC-92-26 (the 24-bit time-truncation bug behind the Dhahran barracks strike) and NSIAD-92-340 (the finding that only about 9% of Army-claimed Scud warhead kills met GAO's "strong evidence" standard). Both reports were, as of this vault's most recent captures, only reachable through mirrors — gao.gov's own PDF assets and product pages have 403'd every fetch route tried across multiple sessions.

- 2026-08-26: IMTEC-92-26's own primary venue (gao.gov, via a Wayback Machine capture — the live URL 403s tooling) is now the cited source_url for both Dhahran claim-notes, replacing the cs.nyu.edu mirror they were promoted on. The re-read also surfaced that GAO's Dhahran investigation was not a relay of the Army's account: investigators independently re-derived the time-conversion fix and ran a live intercept simulation at the Patriot Software Test Facility, Huntsville, over a review period of June 1991–January 1992 ([[claim-gao-imtec-92-26-independently-verified-patriot-time-fix]]) — the same independent-auditor posture NSIAD-92-340 shows in the effectiveness-rate cluster below. Named contributors Michael Blair, Sally Obenski, and Paula Bridickas did the analytic work; Ralph V. Carlone (Assistant Comptroller General) signed the report to Rep. Howard Wolpe, whose subcommittee requested it — distinct GAO officials from Richard Davis, tied to the separate NSIAD-92-340 report below.
- 2026-08-27 (audit, cross-model): correction to the 2026-08-26 entry above — GAO did not "run a live intercept simulation" as a test of the fix. Per Appendix I of IMTEC-92-26, GAO checked the Army's correction "by performing the key calculations needed to account for longer run times" and separately "participated in a simulation" at Huntsville "to become more knowledgeable about how the Patriot system performed in an operational environment" — familiarization, not fix-testing.
