Charles Perrow
American sociologist (1925–2019) at Yale, best known for Normal Accidents: Living with High-Risk Technologies (1984), written substantially in response to the 1979 Three Mile Island partial meltdown. His central argument: systems combining tight coupling with complex, non-linear interactivity will experience catastrophic failures that are structurally inevitable, not merely a symptom of operator error or inadequate safety engineering.
Matters to this vault as the originator of the theory claim-perrow-1984-normal-accident-theory-tight-coupling-complexity states, which John Law applies to Fukushima in 2011 and which Williams & Yampolskiy (arXiv:2104.12582) use directly to frame AI-system catastrophic-failure risk — a 1984 nuclear-safety theory now doing load-bearing work in AI safety.

- 2026-09-14: A direct read of Perrow's own 1984 text remains unobtained after a dedicated session tried every reachable web-tooling route and was blocked on all of them (observation-perrow-1984-primary-text-blocked-across-every-route-attempted). The closest substitute found is Jason Collins's 2017 quote-dense review, which corroborates the tight-coupling/complex-interactivity mechanism's structural shape (claim-collins-2017-review-corroborates-perrow-tight-coupling-and-complex-interactivity) but leaves the "will inevitably" wording-strength question unresolved (claim-collins-2017-review-leaves-perrow-inevitability-wording-unconfirmed).
- 2026-09-14: A same-day sibling session read Todd La Porte's High Reliability Theory (1996) directly, the standard academic counter to Perrow's theory: it accepts Perrow's tight-coupling/complexity structure but denies it forces inevitable catastrophic failure (claim-la-porte-1996-hrt-accepts-perrow-preconditions-denies-inevitability), and distinguishes technical/physical tight coupling from organizational/social tight coupling, an axis Perrow's own vault claim doesn't yet carry (claim-la-porte-hrt-distinguishes-technical-from-social-tight-coupling).
